Iowa woman charged in death at Bondurant assisted living facility

An Ankeny woman has been charged in the January death of 77-year-old Lynne Harriet Stewart. A 30-year-old employee of Courtyard Estates was charged in the case. Catherine Forkpa is charged with dependent adult abuse and intentional reckless abuse.Stewart was staying at the facility in Bondurant, which is an assisted living facility for people with dementia. A cause of death has been released by Polk County authorities. Deputies said Stewart died of hypothermia. She was found outside of the Courtyard Estates at Hawthorne Crossing in subzero temperatures and later died at the hospital on Jan. 21.The low temperature that morning was -9 degrees Fahrenheit. A criminal complaint shows Forkpa did not carry around an iPad, alerting her to any incidents triggering an alarm.That alarm went off for nine hours before Stewart was found.Forkpa’s preliminary hearing is set for March 23rd. She is being held at the Polk County jail on $ 100,000 cash bond. KCCI first reported in January that the Polk County Sherriff’s considered the death “suspicious.” According to reports from the Iowa Department of Inspections and Appeals, Courtyard Estates at Hawthorne Crossing was fined $ 1,500 in summer 2020 for not having a policy or procedure for their alarm system.The facility was later found to be compliant in a 2021 report.Iowa DIA conducted an investigation shortly after Stewart’s death. The results of their investigation have not been released.
